Assoc. Prof. Dr. Barış Metin said that although the most common use of Transcranial Magnetic Stimulation (TMU), which means magnetic stimulation of the brain, is the treatment of depression, there are scientific studies reporting that it provides benefits in many other disorders.

Early intervention is important in stroke

Stating that stroke means a sudden disorder in the blood supply of the brain, Assoc. Prof. Dr. Barış Metin said:

"In stroke, there is no blood supply to a part of the brain or the nutrition of the brain tissue is disrupted as a result of a hemorrhage. In this case, weakness is usually seen in the opposite half of the body. At the same time, loss of skill and rigidity, which I call spasticity, may also occur. In addition to weakness, speech impairment may also occur. When a stroke first develops, we try to reduce the paralysis by intervening as early as possible. However, weakness and stiffness may remain in many patients despite the first intervention. In this case, the rehabilitation process comes into play. Rehabilitation is carried out by specialized physiotherapists and the main goals are to maintain joint range of motion, stimulate the brain to facilitate its re-functioning and restore muscle strength."

TMU can be applied together with physiotherapy

Stating that another treatment method that can be applied together with physiotherapy is TMU, Assoc. Prof. Dr. Barış Metin said, "The most commonly used method in TMU is to suppress the opposite side of the brain, not the damaged one. For example, TMU is given to the right brain in an individual whose left brain is damaged after a stroke. Thus, the functions of the intact right brain are suppressed and the damaged left brain is allowed to recover. In addition to strokes, the same method can also be used in the treatment of speech disorders that develop after stroke."
Stating that there are currently many studies in the medical literature investigating the effectiveness of TMU use after stroke and reporting positive results, Assoc. Prof. Dr. Barış Metin said that the positive results obtained from these studies suggest that this method can be easily used in patients with stroke.

Use with caution in patients with epilepsy

Emphasizing that the most severe reported side effect of TMU is epilepsy crisis, Assoc. Prof. Dr. Barış Metin said, "For this reason, it should be used with caution, especially in people with epilepsy. Other than that, there are no known serious side effects."

Application can be done after the acute period has passed

Stating that TMU treatment can be started after the acute period of stroke, that is, after the first week has passed, Assoc. Prof. Dr. Barış Metin said, "Apart from this, it can also be used in patients who have had a stroke for a long time. Generally, treatment is given daily and lasts for two weeks. However, in some cases, this period can be extended to get more benefit."

Use of physiotherapy in stroke treatment

Stating that interdisciplinary intervention is important in the success of stroke treatment, Assoc. Prof. Dr. Barış Metin said, "Physiotherapy applied by physiotherapists within the team is also important in stroke rehabilitation. Physiotherapy is the functionalization of the nerve-muscle dysfunction seen in the affected body parts by stimulating the brain with physiotherapy and rehabilitation practices. The aim of physiotherapy is to prevent problems that may occur, to regain controlled movement, to make the patient independent again in daily life and to ensure the patient's participation in life again in every aspect."

The patient should be evaluated in detail

Noting that the most important part of the physiotherapy program is the detailed evaluation of the patient, Assoc. Prof. Dr. Barış Metin said, "As a result of the evaluation, physiotherapists decide which rehabilitation method to apply in the light of the information obtained. As a result of interim evaluations, it should be determined whether the current rehabilitation method is effective or not. If the expected improvement is not achieved, the rehabilitation program should be changed or new methods should be added in the light of current information. In this way, it is tried to gain the patient's independence at the highest level in rehabilitation."

Stroke physiotherapy should be started early

Assoc. Prof. Dr. Metin pointed out that it is important to start stroke physiotherapy at the earliest possible time and that the duration and frequency of physiotherapy varies from patient to patient and according to the rehabilitation method to be applied. Pointing out the importance of teamwork, Assoc. Prof. Dr. Barış Metin said, "For effective stroke physiotherapy, it is essential that the physiotherapist and physician work in harmony and that the patient and his/her family actively participate in rehabilitation. In order to prevent new problems that may arise in physiotherapy and for the success of rehabilitation, the individuals who make up the team should act together and exchange ideas."
