The Treatment Motivation Questionnaire (TMA) is a 26-item self-report questionnaire developed by Ryan et al. (1995) and designed to assess people's reasons for participating in and staying in alcohol/substance dependence treatment.

It is a 5-point Likert-type scale in which questions are answered from "Strongly disagree" to "Strongly agree". Factor analysis and item analyses were conducted with 109 consecutive applicants to the outpatient unit of an alcohol and drug treatment center.

Factor analysis showed that the scale contained 4 identifiable factors: (1) intrinsic motivation (IM - internalized motivation, although not entirely self-determined, (2) extrinsic motivation (DM) - a choice other than seeking treatment.feeling powerless and external pressure to stay in treatment, (3) interpersonal help-seeking (ITS) - motivation to share the case's problems with others, and (4) treatment mistrust (TG) - expectations about the course of treatment.

Ryan et al.3 then applied the TMA to a second sample of alcohol abuse treatment-seekers (n=98). The TMA was highly correlated with clinician-rated general impairment, general motivation, identified motivation, internalized motivation, and extrinsic motivation, suggesting its construct validity.
