Nursing is a profession focused on the care of individuals, families, and communities. Nurses working within this system provide the same level of care and information not only to individuals but also to their families.

In addition to Psychiatric Nursing, we provide nursing care in specific areas such as Intensive Care Nursing, Operating Room Nursing, and Surgical Nursing, all within the framework of Patient Safety principles.

Professional nursing care serves individuals and society from birth to death. Throughout this service, nurses support patients and their families and provide care addressing their physiological, psychological, social, spiritual, and intellectual needs.

1. Processes included in Patient Safety Standards:

• Effective Communication: Patient care is delivered through collaboration among all healthcare professionals, guided by a teamwork approach and focused on patient treatment and care. Patients and their families are informed using empathetic and professional communication skills.
• Fall Prevention: To prevent falls and any resulting complications, our nurses conduct internationally recognized risk assessments during the initial admission process. For patients at high risk of falling, caregiver services may be provided, and our nurses closely monitor patients using our 24-hour video surveillance system. Our goal is to prevent falls not only during their stay with us but also after discharge through education and information.
• Patient Identification Verification: When performing examinations, treatments, and care for our patients, we conduct an identification verification process. Identification is verified using criteria such as first and last name, protocol number, and date of birth (day/month/year). As a general rule, verifying any two of these criteria is sufficient.
• Infection Prevention: Although it may seem simple, handwashing is a crucial aspect of infection prevention. Hands are washed before and after every procedure performed on a patient. Our nurses provide “Handwashing Training” to patients and their families regarding handwashing during both the hospital admission and discharge processes.
• Medication Administration: To ensure the safety of high-risk medications and all other medications, they are entered into the Stockart Medication Management System. Nurses registered in the system dispense medications specifically for each patient. We administer medications in accordance with the 8+1 Principle and record them in the automation system.
• Right Patient/Right Side/Right Procedure in Surgery: In the Surgical Clinic, the surgical site is marked, and the patient’s identity is verified. Similarly, when the patient is received in the operating room and immediately before entering surgery, the patient’s identity is verified again before the patient is taken into surgery. This is known as the “Time Out” process.
• Safe Blood Transfusions: Before a blood transfusion is administered, a cross-match is performed. Patient identification principles are applied; all information is verified and documented before the blood is administered to the patient, and the patient is closely monitored for any potential reactions.

2. We Measure the Nursing Care We Provide.

• To ensure that nursing care is delivered correctly to our patients, we collect and analyze care data in collaboration with the quality unit. Based on these analyses, we plan training sessions and undertake initiatives to improve the quality of nursing care.
• Our Care Quality Indicators include: Pressure Ulcer Rates, Patient Fall Rates, IV Complication Rates, Infection Rates, Employee Satisfaction, and Patient Satisfaction

3. We document all care provided to our patients in accordance with the nursing process, using the stages of assessment, planning, implementation, and evaluation.

• The nursing process ensures the application of scientific knowledge, a systematic approach to problems, standardization of care, quality of care, and evaluation of care.
• Using NANDA Nursing Diagnoses, we plan, implement, and evaluate daily nursing care tailored to each patient. We document all nursing care provided to our patients in our automated system.

4. In our Surgical and Intensive Care Units, we provide “Consultation-Liaison Psychiatric Nursing” services to offer psychological support to patients and their families.

• In addition to physical health, psychological processes are also important for patients and their loved ones.

• Currently, psychiatric nurses work not only in the psychiatric clinics and wards of existing psychiatric hospitals but also in schools, prisons, and orphanages.
• In these settings, he assumes and fulfills various roles, such as counselor, caregiver-therapist, educator, consultant, researcher, and manager.
• In the counseling role, psychiatric nurses conduct work related to supporting mental health and preventing mental illnesses; in primary prevention, they apply this role to at-risk groups in the community. Examples include newlywed couples, families with newborns, those with disabled relatives, and individuals experiencing a midlife crisis.
• Within the Caregiver-Therapist Role: By utilizing therapeutic communication techniques, he identifies the needs of individuals and the community; based on these, he establishes nursing diagnoses, plans and implements nursing care tailored to these diagnoses, and evaluates the outcomes.
• In the Educational Role: He plans and delivers in-service training to all members of the community—particularly patients and their families—as well as to healthcare professionals in the institution where he works. He also continues to provide these educational programs at nursing schools.
• Consultant Role: The psychiatric nurse provides consultation to fellow nurses working outside of psychiatric clinics on topics related to psychiatric approaches and patient-nurse relationships. Examples include intensive care patients and their families, oncology patients and their families, and maternity clinics.
• Researcher Role: By conducting research on psychiatric approaches and care principles within the community and existing systems, he evaluates the current system, identifies deficiencies, makes recommendations, and publishes these studies with statistical data.
• Managerial Role: The psychiatric nurse assumes a leadership role using contemporary approaches.

PSYCHIATRIC NURSING AT NPİSTANBUL BRAIN HOSPITAL

• Within the scope of these roles, our psychiatric nurses primarily adopt a caregiving approach toward our inpatients.
• The Therapeutic Process in Psychiatric Nursing: It begins before our patients are admitted to the hospital; based on information we receive from their families, immediate relatives, and the attending physician, we make nursing assessments, plan care, define processes, and implement them.
• During the nursing assessment, coping strategies are examined, and psychiatric and mental status evaluations are conducted.
• Mood and affect, perceptions, attention, consciousness-memory-orientation, thoughts, behaviors, and activities are assessed. Communication styles are also evaluated. The patient is asked whether he has an understanding of the nature of his illness, and finally, a Preliminary Psychiatric Nursing Diagnosis is made along with the interviewer’s commentary. Based on this, psychiatric nursing approaches are evaluated, determined, planned, and implemented.
• Psychiatric nursing care processes are implemented for our patients in accordance with these diagnostic criteria. Along with medication treatments and adjunctive diagnostic and therapeutic methods, occupational therapy programs are planned and implemented to help patients continue their daily living activities. “Good morning” meetings and reading sessions are held with them.
• Arts and crafts, occupational therapy, music therapy, ebru, painting, and yoga sessions are planned and conducted jointly under the supervision of psychiatric nurses and specialized instructors. Participation in and planning of these activities are primarily managed by the psychiatric nurse.
